The investigation showed that 17-year-old Baylee E. Blackburn, of Paducah, was operating a 2006 Mazda passenger car when she was westbound on Lebanon Church Road and was attempting to cross the intersection of Old US45. Miss Blackburn pulled into the path of 46-year-old Edward W. Gagnon, of Paducah, that was operating a 2001 Ford F-150 South bound on Old US45. Mr. Gagnon attempted to avoid the collision but was unable to maneuver safely to prevent from striking the rear bumper of Blackburns vehicle. Blackburns vehicle spun 180 degrees striking a third vehicle that was stopped at the stop sign, in the East bound lane, on Lebanon Church Road before coming to rest in a ditch. The third vehicle was a 2006 Dodge Durango operated by 43yoa Michelle Grisham, of Paducah.

Miss Blackburn was transported by Mercy Regional Ambulance Service to an area hospital for what appeared to be only minor injuries.
