fatal collision 1 300SLAUGHTERS, Ky. (4/9/14) — A Madisonville man has died and two people were injured during an early morning vehicle collision on the Pennyrile Parkway, Wednesday, April 9.

According to a Kentucky State Police report, Cody W. Allen, 22, of Madisonville was driving a 2000 Ford Explorer at 6:52 a.m. south on the parkway when he lost control of the vehicle. The vehicle traveled into the median where it began to overturn, the report said.

The vehicle continued to overturn across both northbound lanes, leaving the roadway and coming to rest upright off the northbound side of the road.

According to the report, Allen was not wearing a seatbelt and was ejected from the vehicle. He was transported via ambulance to Baptist Health Madisonville, where he was pronounced dead by Hopkins County Coroner Dennis Mayfield.

Thomas W. Hawkins, 22, of Hanson was a passenger in the vehicle and was not wearing a seatbelt. He was also ejected from the vehicle. Another passenger, Amber A. Harrison, 20, of Henderson was wearing a seatbelt. Both passengers were flown from the scene and taken to St. Mary's in Evansville, Ind.

Alcohol is a suspected factor in the collision, according to the report.

Trooper John Komar is continuing the investigation.

Hanson Volunteer Fire Department, Manitou Volunteer Fire Department, Sebree Volunteer Fire Department and Medical Center Ambulance Service assisted at the scene.
