The preliminary investigation revealed that Michael H. Phebus, age 25 of Dawson Springs, KY, was operating a 2000 GMC Sierra pickup truck eastbound on KY 672 when his vehicle struck a patch of ice on the roadway causing him to lose control.

The truck exited off the right side of the roadway. Phebus overcorrected and crossed both lanes. It exited the roadway and traveled into a ditch. The front of the truck struck the ground causing it to overturn onto its top. It came to rest on its top in a ditch off the westbound side of the road.

Mr. Phebus was given a ride by a passerby to Dawson Springs where he was treated by Medical Center Ambulance Service and then transported via ambulance to Baptist Health Hospital. Mr. Phebus was not wearing a seatbelt.
