HENDERSON, Ky. (5/13/19) — A 21-year-old Henderson resident lost his life Sunday in a mobile home fire despite heroic efforts to rescue him.

According to a Henderson Fire Department report released by the city this morning, Zachary Oliver was identified as the victim.

The State Fire Marshal’s Office ruled the origin of the fire as “undetermined.” There were no smoke detectors in the home, according to authorities.

HFD responded to the structure fire 5:52 p.m. at 1599 N. Kentucky Ave., Lot No. 2, originally reported that there was a person trapped inside. Before fire crews got to the scene, an off-duty firefighter drove by and saw the fire.

“He made several attempts to make entry, but was driven back by the heat and smoke,” HFD Chief Scott Foreman said in a statement. The firefighter was not injured.

After the fire was quickly knocked down, the city reported Oliver was found deceased inside the home. Henderson County Coroner’s Office and Kentucky State Fire Marshal’s Office were called out to investigate.

Henderson city officials indicated one firefighter received a minor injury during the incident, and was taken to Methodist Hospital for treatment, and then returned to duty a short time later.

An autopsy is scheduled for Oliver's body this morning at the Kentucky Medical Examiner’s Office in Madisonville.

Henderson County Emergency Management Agency also assisted.

HENDERSON, Ky. (5/12/19) — Henderson Fire Department was called to the scene of a mobile home fire just before 6 p.m. on Kentucky Avenue in Henderson. EMS is also at the home.

When the report was first dispatched it indicated that someone was still inside and when fire crews arrived, they found the home fully engulfed.

Witnesses at the scene said an off-duty Henderson firefighter driving down Green Street noticed smoke coming from the mobile home and went into the home three times trying to rescue the victim but was unable to locate anyone.

The coroner was summoned to the scene around 6:30 p.m.

Henderson Fire Department has requested assistance from the State Fire Marshals Office for an investigation.
